If you are receiving lessons from one of our teachers, and would like to enroll an additional student with the same teacher, please complete and submit the form below. You must be the parent or legal guardian of the new student.
If you wish to stop lessons until further notice, please fill out and submit the form through this link. The form includes an optional exit survery. Any additional feedback you can provide about your music lesson experience is greatly appreciated!
